ASSESSMENTS & REPORTS

New Clients:

As part of our intake process, all new clients (parents) are required to undergo an Initial Consultation with one of our Occupational Therapists. This consultation allows us to gather essential information about the child's history, concerns, and goals for therapy.

Diagnostic Assessment:

A Comprehensive Assessment is conducted for each client seeking a diagnosis prior to initiating a treatment plan or to contribute valuable information towards a potential diagnosis. A detailed report is provided following the assessment, and charges are determined based on the time required to complete it.

Functional Capacity Assessments:

A Functional Capacity Assessment is required by NDIS.  It enables OT’s to determine a child’s developmental needs across a variety of environments, developmental stages and ages.

It also enables the OT to consider what types of equipment each child may need over a period of time.

Equipment provision is required to undergo a rigorous process that includes trials, quotes and reported substantiation of needs, based on the results of the trials

Clients with Previous OT Assessment Reports:
(within the past 6 months)

If your child has undergone an Occupational Therapy assessment elsewhere within the last 6 months, we kindly request that you provide us with a copy of the OT report during your Initial Consultation at SSOT. This enables us to review the assessment findings and consider the appropriate inclusion of your child into our group programs or individual therapy sessions.

Please note:

  • Additional assessment may be necessary based on the specific needs, time frames and goals identified during the Initial Consultation or review of previous assessment reports.

  • Our team is committed to ensuring that all assessments are conducted with thoroughness and accuracy, in alignment with best practices in paediatric occupational therapy.

  • We understand the importance of timely and comprehensive assessment in guiding our therapeutic interventions and supporting the developmental needs of each child.

If you have any questions or require further clarification regarding our assessment procedures,
fees, or scheduling, please do not hesitate to contact us. We are dedicated to providing
exceptional care and support to every child and family we serve.

OUR PROCESS:

Before commencing any treatment regimen, we conduct a thorough Comprehensive Assessment for every new client. This assessment serves as the foundation for developing a personalised and effective intervention plan.

NEXT STEPS:

Following the completion of a Functional Capacity Assessment (above), our skilled team analyses and interprets the gathered information to generate a detailed report.

This report includes:

  • A concise overview of the child's strengths, areas of concern, and identified needs based on the assessment results.

  • Individualised recommendations tailored to address the child's specific goals and challenges, including suggested interventions, strategies, and support services.

  • Opportunities for collaboration with parents, caregivers, educators, and other professionals involved in the child's care to ensure primary goals are prioritised and that a team approach supports each child’s goals and developmental stage.

  • In some cases, specific equipment is warranted to service the specific needs of a child. We prefer to trail equipment wherever possible to determine the ‘just right fit’ for each child. Where appropriate ( for additional cost), ‘Letters of Support’ can be provided to NDIS regarding the benefit of particular equipment that has been trialed with an individual child and proven to be of benefit to that particular child.

Our commitment to evidence-based practice (EBP’s) and personalised care drives our assessment process, intervention and report writing.

We also spend time in therapy with individuals, working on Practice Based Evidence (PBE) using trials with equipment etc to determine what works in each child’s unique circumstances. We strive to provide clear, actionable recommendations that empower families and professionals to support the child's development effectively.
